Peninsula Community Health Services (PCHS)
is a non-profit organization with the mission
of “strengthening our community by providing
accessible, affordable health care.” Our Patient
Centered Medical Home Model includes
Cottonwood Medical and Behavioral Health
Center and Aspen Dental Center. Our team
approach to health care combines physical
and behavioral health care through caring
providers and supportive staff.
This is our 4th annual Love Lights program
to benefit patients who have health care needs
and do not have the resources to meet those
needs, such as: medicines, transportation, and
specialty care.
We hope that you will join our Board of
Directors and staff in donating to the Love
Lights Patient Assistance Program. Please
help us provide support to our neighbors who
are dealing with difficult health and financial
challenges.
We invite you to sponsor a light or lights to
commemorate and honor a friend or loved
one. For $5.00 each, you can sponsor a light
that will shine throughout the winter months
as a representation of your gift. It is easy to
participate in the Love Lights program. Simply
detach the donation form and complete the
information requested. Choose the number of
Love Lights you wish to sponsor and return the
form with your tax deductible donation to the

The tree lighting ceremony will be held at
5 p.m., on December 5th, 2012 in the first
floor reception area of our 230 E. Marydale
location.
