Profile:
Dr. McIntosh is a family practice physician who received her medical degree from McGill University in Montreal, Canada. She provides a full range of family practice from neonates to geriatrics, workers compensation cases, and well person physicals including occupational licensing physicals.

She has a special interest in developmental pediatrics and is part of the FAS Diagnostic Team for Alaska which locally holds clinics through Frontier Community Services.

She believes that her medical practice is a partnership with her clients in which she educates, empowers and advocates for them. In return, she believes they educate and inspire her as a physician.

Dr. McIntosh is married and has three children, two adult and one teenager. She volunteers for the Special Olympics and is an active participant in her church. Dr. McIntosh loves the Kenai Peninsula and is grateful to live here.

Colette Reahl, MD

I began practicing as a General Pediatrician in 2011. My professional degrees were completed at the University of Florida and the University of Miami Miller School of Medicine.  

The settings in which I care for children are: the nursery, outpatient clinic, and hospital. The emphasis of my practice is promoting healthy lifestyle and preventative health-care. Being a General Pediatrician allows me the privilege to care for and advocate for our future generation. By working with PCHS, I am honored to have the opportunity to serve our community and impact our future by helping all children who present for our services, including those who have difficulties obtaining care.  

Although I am originally from Jamaica I have grown to love Alaska. My husband and I love to spend time in nature and are excited about the many ways in which we will be able to enjoy the great Alaskan outdoors throughout the beautiful Kenai Peninsula. Apart from work, I participate in Christian Ministries, Local and Foreign Missions, and Music Education. Of all the things I love to do working and caring for children is my favorite. You will always find a smile on my face because I find joy while using my experience and knowledge to care for children. Working as a Pediatrician is not just my job, but my passion. I look forward to meeting and working with all of the precious children of the Central Peninsula.

Dr. Wiseman is a board certified family medicine physician who provides comprehensive family medicine care to all ages. She particularly enjoys women’s health, children and preventative medicine.
 
She believes health and medicine act as a continuous dynamic between an individual’s physical, mental and emotional well being and feels family medicine helps to promote all of these aspects rather than one over the other. She is privileged that patients allow her to be an integral part of their lives and families. She considers each patient experience to be a learning opportunity for both her patients and herself.
 
Dr. Wiseman is married with two young children and recently relocated to the Kenai Peninsula from the Midwest. She grew up in Texas where she met her husband. Profile:
Heidi Koslo is a family nurse practitioner and lactation consultant whose healthcare philosophy is holistic and family-centered. She started her healthcare career providing mother-to-mother breastfeeding support, and moved on to providing nutrition and lactation support through the WIC program in Wasilla, AK in 2004.
As a Registered Nurse since 2007, she has worked in Maternal-Child and high-risk Perinatal hospital units in South Carolina and Alaska. In 2012 she became an Advanced Practice Registered Nurse (APRN) and has spent the past two years serving patients across the life-span in the Alaska Native and American Veteran populations (by special agreement) through Southcentral Foundation.
Heidi is happily married to a wonderful husband and they have a teenage daughter, two dogs, and a cat. She enjoys spending time with her family, reading, beading, and outdoor pursuits such as kayaking and hiking.

Tammy LaFrancois-DeGraeve, MSN, FNP-BC

Profile: Tammy is a board certified family nurse practitioner with over 25yrs of health care experience and 17 yrs as a nurse practitioner. She spent the first 22 years of her medical career serving on active duty in the US Army and retired in 2009. She has a very broad experience base and truly enjoys the full spectrum of family practice because it spans a patient’s entire life allowing long standing patient relationships. She believes in listening to the individual needs of each patient/family to develop a comprehensive, individualized plan of care. She enjoys teaching patients and family members about their conditions and is a strong patient advocate.
Tammy has three sons and one granddaughter. She enjoys a healthy, active lifestyle while spending time with family and friends. She especially enjoys running, skiing, ice hockey, cooking, travel and the great outdoors.

Profile: Carlos A. Valenzuela is a family nurse practitioner who has worked in orthopedics, family practice, and emergency care settings for over 12 years.

Dr Cannava was raised in Soldotna, Alaska. He completed college undergraduate work in Oregon, all Masters coursework at University of Alaska, Anchorage and finally earned Masters and Doctorate degrees in Clinical Psychology in California. During his school years Dr Cannava worked and trained at both inpatient and outpatient settings in Alaska and California. Dr. Cannava returned to Soldotna full time in 2000 and has worked at community health centers in both Soldotna and Seward since. Dr. Cannava works primarily with teens, adults, couples and families.

Corey Mihailovich was born and raised in beautiful Butte, MT. He received his B.S. in zoology from Washington State University. Profile:
Bill grew up in rural Florida with a dream of living in Alaska. After completing dental school, Bill moved to Alaska to work as a dentist. He worked in Dillingham for five years and then Homer for six years.
In 2014 Bill moved his family to Oamaru, New Zealand where he worked as a dentist for a year. Bill and his family had a wonderful time on that trip and met a lot of fabulous people. As is true of all big adventures, it was time for the family to come home and they moved back to the Kenai Peninsula in 2015.
Bill and his wife Sarah have two children. They enjoy fishing, boating, camping and aviation. They look forward to raising their family on the Kenai Peninsula.

Zanyce (Zee) Berg believes all dental experiences can / and should be positive. She is passionate about public health and having the opportunity to serve those who may not otherwise have access to care.

Zee began her career with Aspen Dental back in 2003 as a volunteer while attending college. In 2004 Zee was hired by Aspen Dental Clinic as an Administrative Assistant. In 2005 she was excited to be accepted to University of Alaska Anchorage's (UAA) Dental Hygiene Program and as part of her schooling, was required to complete an externship. Aspen again was happy to have her back for this requirement and she completed it Summer 2006. Zee was awarded an AAS degree from from UAA, May 2007. She was happy to professionally join the Aspen Dental team in 2008 and was excited to begin her dental career in the capacity of a dental hygienist.

Zee has four children of whom she adores and enjoys Aces Hockey!

Profile :  Michael J. Perry is a board certified Family Psychiatric Mental Health Nurse Practitioner born and raised in upstate New York. He began his nursing career 23 years ago when he received his license practical nurse degree from BOCES in New Hartford, New York. He then earned his Associates in Applied Science, Nursing from Mohawk Valley Community College in Utica, New York, before completing his Bachelor of Science, Nursing from SUNY Institute of Technology also in Utica, New York. Michael then completed his Masters of Science, Nursing in the Family Psychiatric Mental Health Nurse Practitioner Program as SUNY Upstate Medical University, Syracuse, New York. Mr. Perry is currently enrolled in Chatham University, Pittsburgh, PA Doctor of Nursing Program and anticipates graduating in 2015 with his Doctor of Nursing Practice.
Before coming to PCHS, Mr. Perry worked at Claxton-Hepburn Medical Center in Ogdensburg, New York as an acute care inpatient and outpatient Psychiatric Nurse Practitioner, as well as providing psychiatric consults and medication management to the hospital's medical floor, emergency department and outside agency servicing seniors.
Prior to his time at Claxton-Hepburn, Mr. Perry practiced at Mary Imogene Bassett Medical Center in Cooperstown, New York as an acute care inpatient Psychiatric Nurse Practitioner.
In addition to his work as a psychiatric nurse practitioner, Mr. Perry has an extensive background in psychiatric nursing including staff nurse, nurse administrator and program coordinator for the state of New York.
In his recreational time, Michael enjoys hunting, fishing, playing guitar and road trips with his wife. Coming to Alaska fulfills a dream Michael has had since he was a child and he looks forward to serving the people of this community.

Mary enjoys having a sense of humor, fishing and travel. Mary was born and raised in and around New York City but rarely sounds like a New Yorker. She has lived in three countries and six states. She has traveled extensively and has worked with people from Russia, Mexico, and Viet Nam and has had the privilege of learning about people from many different backgrounds including Irish, Cup’ik, Yup’ik and Chamorro. 

Mary has trained with experts who were ‘way smarter’ than her and has learned to assist her clients in living the lives they choose. Across the years her clients and their journeys toward wellness have been her most esteemed teachers. Mary strongly believes that people can and do get well.

She has worked in the Chemical Dependency field for 16 years, and recently became a Behavioral Health Clinician at PCHS.

Michele has been married to Steve for almost 42 years, and they have 4 grown children, 11 grandchildren, and one great-grandchild. Grand parenting is her hobby and the joy of her life; with never a dull moment! Life is full of joy!
Michele enjoys the work she has done during her career and is looking forward to many more years serving her community. Her favorite part of counseling is facilitating the Co-Occurring groups and interacting with each client on a personal level.

Kristina was born and raised in Slovakia and she came to Alaska in 2003 as an Au-pair. She considers Alaska her home; however she visits her homeland and other European states, every few years. In her free time she enjoys spending time with her family, biking, hiking, and spending time on the water. 
At PCHS, Kristina works with both; adults and children. She helps adults achieve and sustain recovery from drugs and alcohol using evidence based practices, and offering individual, positive, and non-judgmental approach. Kristina practices Person-Centered Approach developed by Dr. Carl Rogers, along with Cognitive-Behavioral Therapy, Acceptance and Commitment Therapy, and Mindfulness. 
To help children become the best version of self, Kristina strives to offer safe environment, where children are accepted as they come. Through Play and Narrative therapy, children are able to express and experience their emotions and discover their own abilities that can help them resolve challenges they are currently experiencing.

Michelle is a behavioral health clinician specializing in adult holistic care of the whole person – mind, body, and spirit. Michelle has worked with a diverse adult population since 2010. Kelli is a Clinical Team Leader who provides mental and behavioral health counseling to youth, children, adolescents and their families across the Peninsula who are struggling with various life issues across domains. She aims to create a trusting therapeutic relationship to assist youth on their journey towards holistic health and wellness by developing a skill set that increases their ability to cope with on going stressors related to underlying traumatic events that are contributing to current life struggles. Kelli has a special interest in working with adolescents to develop a network of both internal and external resources to guide them in their journey towards wellness. Kelli enjoys having a sense of humor, playing and coaching softball, and being an active member of the community aiming to provide a sense of hope to all those she encounters.

Born and raised in Utah. Moved to Alaska in 2002 as an elementary school counselor for Cordova School District. Moved to Clam Gulch in 2007 worked as a Behavior Specialist and then a principal for KPBS. Took a job with LYSD as a Positive Behavior Interventions and Support Coordinator in 2012. Bachelor degrees in Special Education and Mathematics, Masters in Counseling and School Psychology, Certifications in Educational Leadership, and recently received my Superintendent endorsement. I have been trained in Parenting the Love and Logic Way (facilitator), Parenting with Love and Limits (Therapist), Empowering Parents, Stone Soup PBIS, and Safe and Civil Schools.

Education and Training: 
Jessica received a Masters of Science Degree in, Education in Counseling (MS), from the State University of Wyoming. She has been awarded the titled of National Certified Counselor (NCC) by the National Board of Certified Counselors. She is currently working to satisfy the requirements for her Licensed Professional Counselor Certification (LPC). She anticipates becoming eligible to apply for licensure in January of 2016. She has earned her Chemical Dependency Counselor I certification (CDC1) from the Alaska Commission for Behavioral Health. Jessica has also undergone extensive training to become eligible to facilitate the Parenting with Love and Limits (PLL) Model Program, which is recognized through OJJDP and SAMSHA’s National Registry of Evidenced-Based Programs and Practices.

Jessica has a broad knowledge of human behavior, family dynamics, crisis intervention, group procedures, ethics, theories, diagnosis, and research. She has additional training and experience with addictions, play therapy and working with children of all ages. She is currently devoting her time to the Parenting with Love and Limits (PLL) program, which offers an evidence-based family education, skills-building and therapeutic intervention model to at risk youth and their families.

Profile: 
 Mary Jo is a Clinical Team Leader who assists youth and their families by providing mental and behavioral health services as needed to help them navigate obstacles that try to prevent them from reaching their goals and living a happy, healthy life. She believes that her role as a counselor is similar to that of a GPS—to guide clients toward successfully arriving at the destinations they wish to make it to. She believes that anyone can make positive changes in their life at any stage or age.

A lifelong Alaskan who was born and raised on the Kenai Peninsula, Mary Jo enjoys fishing, taking time out to appreciate the natural beauty we’re surrounded by, photography, softball, spending time with loved ones (two and four-legged), and is active in the community and her church. She has many titles, but one of her favorites is grandma.
